Grades 9-12 must submit an application which should also include qualification, you should only use 5 Fall semester grades and a community service verification form that documents 5 service hours from 8/15/19 – 1/23/20. The deadline to submit both forms is on Friday January 24, 2020 in the CSF box in Guidance Office.
